Recurrent urinary tract infections (UTIs) are a common and frustrating problem, particularly for women, but also for men and older adults. A recurrent UTI is defined as two or more infections within six months, or three or more within a year. Fortunately, modern medicine now offers a wide range of diagnostic and therapeutic options – far beyond repeated courses of antibiotics.

A key step is an accurate diagnosis – not every urinary symptom is automatically an infection. Proper urine analysis and cultures are essential to identify the exact bacteria and to avoid unnecessary antibiotic use. In patients with frequent recurrences, further investigations may be recommended, such as ultrasound, bladder function testing, or cystoscopy, to rule out underlying causes including bladder emptying problems, stones, anatomical changes, or chronic inflammation.

When it comes to treatment, current guidelines promote an individualised approach. While antibiotics remain important for acute infections, they should be used carefully and in a targeted manner. There has been an increasing attention given to non-antibiotic strategies, including behavioural measures (adequate hydration, bladder training), vaginal oestrogen therapy for postmenopausal women, immunoactive prophylaxis, and selected supplements that can reduce bacterial adherence to the bladder wall. In some cases, tailored long-term prevention strategies are more effective than repeated short-term treatments.
